Do not submit patient medical records, diagnoses, treatment notes, medication details, insurance records, or other protected health information (PHI) through this form. Providers are responsible for their own intake, HIPAA/privacy obligations, clinical communications, consent, records, and treatment services.
Licenses and professional credentials may be independently verified before approval. Submitting an application does not guarantee network acceptance or compensated referral status.
