Recovery groups, ministries, churches, nonprofits, community organizations, treatment providers, employers, transportation partners, housing resources, and other organizations can apply to work with our network. Approved partners may receive a unique tracked link or QR pathway so referrals can be attributed accurately. Where a written compensation agreement applies, eligible confirmed referrals can be tracked separately from ordinary resource connections.
Community partner pathway: Apply → review and verification → written partner terms where applicable → unique partner link or QR → transparent referral attribution → eligible compensation only when the agreed requirements are met.
Dedicated partner page hosting: New partners who request or receive a dedicated Careziaspace / Rebound partner page agree to a $20/month minimum Partner Page Hosting & Maintenance fee. This covers basic hosting and keeping the dedicated partner page available. Major redesigns, custom features, integrations, unusually high usage, or additional infrastructure may carry separate charges, disclosed and agreed to in advance. Existing partners are not automatically enrolled or charged; their arrangements are handled separately.
Privacy & referral standard: Careziaspace Technologies / Rebound.Solutions is a navigation and referral platform, not the treating provider or patient medical-record system. Do not submit patient medical records, diagnoses, treatment notes, medication details, insurance records, or other protected health information (PHI) through this form. Providers are responsible for their own intake, HIPAA/privacy obligations, clinical communications, consent, records, and treatment services.
Rebound Solutions may request licensing, accreditation, insurance, references, policies, service details, and written agreements before any organization is displayed as verified.
